Adams County, Colorado local guide: This county or county-equivalent page applies the state and national guidance and official resources below to people looking for accident information in Adams County, Colorado. The agency responsible for a police report or records request can depend on where the incident occurred and which agency investigated it. Confirm the agency shown on the report or incident information before requesting a record.
Short answer: In Colorado, a driver involved in a traffic Uber accident with any property damage, injury, serious bodily injury, or death must give immediate notice to the nearest authorized police authority after completing the stop-and-information duties in the traffic code. That does not guarantee that an officer will investigate or make a police report. If no officer is called to the scene, Colorado’s Department of Revenue (DOR) offers an online, driver-submitted counter report. A counter report is kept for records purposes and is not investigated by law enforcement. Colorado Revised Statutes, Title 42, §§ 42-4-1602–1606; Colorado DMV Uber accident-record instructions
Immediate checklist
- Stop, exchange required information, and give immediate notice to police. Colorado’s reporting statute applies to Uber accidents with property damage as well as Uber accidents involving injury or death. If police direct a driver to return to and remain at the scene, the statute says to do so until the investigation is complete. C.R.S. § 42-4-1606
- Keep the basics. Save the date, time, location, other driver’s contact and insurance information, vehicle plate/VIN if available, responding agency, and any case or report number. Colorado requires drivers to exchange name, address, vehicle registration number, and—on request—driver license information in covered Uber accidents. C.R.S. § 42-4-1603
- If an officer was not called, use the DOR online counter-report option. It is a driver report, not an officer investigation. File a Uber accident report online through Colorado DMV
- If an officer responded, keep the report number and wait for processing before ordering a copy. DOR asks requesters to allow 90 days for the law-enforcement report to be received and entered; Colorado State Patrol (CSP) says its reports may take up to 10 working days after the investigation process. Who reports what in Colorado?
| Report or notice | Who handles it | What the official source says |
|---|---|---|
| Immediate notice to police | Driver, or a capable occupant if the driver is physically unable | The driver must give immediate notice to the nearest authorized police authority for a traffic Uber accident involving injury, death, or any property damage. A capable occupant must give or cause notice when the driver cannot. C.R.S. §§ 42-4-1606–1607 |
| Officer investigation / traffic Uber accident report | Law-enforcement officer receiving notice or investigating within the officer’s jurisdiction | Officers must submit reports of notified or investigated traffic Uber accidents to the department within five days after receiving information or completing the investigation. The law has a narrow officer-report exception for certain no-injury motor-vehicle Uber accidents when the officer reasonably believes damage to any one person’s property does not exceed $1,000; an officer still must investigate and report if a participant specifically requests it or cannot show proof of insurance. This exception concerns the officer’s report—it does not narrow the driver’s immediate-notice duty. C.R.S. § 42-4-1606(4) |
| DOR online counter report | Individual driver when no officer was called | The DMV says a person may file online. The report is not investigated by law enforcement and is kept for record purposes only. It is called a counter report. Colorado DMV |
| Supplemental report or DOR request | Driver or witness, if required by the department | The statute permits the department to require supplemental reports when an original report is insufficient and to require witnesses to report. The DMV says that when an officer gathered the needed information at the scene, no additional form is needed unless DOR sends a letter telling the person what to do. C.R.S. § 42-4-1606(3); DMV |
| Insurance notice / financial-responsibility item | Driver and insurer; in the circumstance identified by CSP, the driver | This is separate from reporting to police or DOR. CSP advises notifying an insurer or agent. Its page also says that a person who did not give a trooper current insurance information must complete a financial-responsibility form within 10 days; that instruction is specific to the circumstance CSP describes. CSP Uber accident information |
A driver-submitted counter report and an officer’s report serve different purposes. Under C.R.S. § 42-4-1610, reports required from drivers, owners, or involved people are confidential for departmental use, while investigation reports made by sheriffs, police officers, coroners, or other peace officers are not made confidential by that section and are subject to the record-access provisions in § 42-1-206. That difference does not turn a counter report into a police investigation.
How to request a Uber accident record
DOR/DMV route for a Uber accident record
Colorado DMV’s Uber accident-record page directs a person seeking their own report to submit DR 2559, Permission to Release Driver Records to Self or Another Person, with the required fee. To request another person’s Uber accident report, the page requires either that person’s signed DR 2559 authorization or DR 2489, Requestor Release and Affidavit of Intended Use, plus the fee. These restrictions reflect the Driver’s Privacy Protection Act and Colorado privacy provisions. DMV Uber accident-record instructions; DR 2559; DR 2489
The current DR 2559 and DR 2489 forms list a $9.25 non-certified search fee and a $10.25 certified search fee. Confirm the required record type and current form instructions before submitting payment. DOR says requests may be mailed or handled by appointment at its Westgate location; its Uber accident-record page says Uber accident and other records are available only at 3265 S. Wadsworth Blvd. #3A, Lakewood rather than ordinary driver-license offices. DMV Uber accident-record instructions
For a counter report, DR 2559 specifically asks for the confirmation number, Uber accident date, driver identity, and driver signature. The DR 2489 Uber accident-report section asks for the driver information plus the police department, report or case number, VIN, date, location, and city/county. Gather those identifiers before requesting a copy. DR 2559; DR 2489
DOR’s webpage and the current request forms display different post-office-box addresses for mailed records requests. Use the instructions on the most current form and DMV Uber accident-record page when preparing a mailing rather than relying on an older copy.
Colorado State Patrol route—only for CSP-investigated Uber accidents
For a Uber accident investigated by CSP, use the CSP Central Records Unit request portal. CSP asks for the driver’s name, Uber accident date, case report number, and location. A CSP basic Uber accident report is the trooper’s traffic-Uber accident synopsis; CSP says it does not include witness statements, digital media, or other parts of the investigation. CSP Uber accident information; CSP Central Records Unit
CSP says requests go through its portal, payment may be made by check, money order, or credit card, and a completed investigation may still take up to 10 working days through submittal, review, corrections, and approval. Its published schedule lists an initial criminal-justice record of up to 10 pages at $5, then $0.25 per one-sided page; review, production, or redaction time can add charges. CSP also states that it will not release CCJRA records unless the requester signs that the records will not be used for direct business solicitation for pecuniary gain. CSP fee schedule
If the report is unavailable or another agency responded
Do not assume CSP keeps every Colorado report. CSP says it does not have copies of driver-submitted online reports; those are filed with DOR. CDOT says that an officer’s DR3447 is sent to DOR, where the official legal record is maintained in DRIVES, while CDOT’s safety database receives data for Uber accidents excluding private-property Uber accidents and counter reports. CDOT also says individual Uber accident-report requests should go to DOR and that original copies may be available directly from the responding law-enforcement agency. CSP; CDOT Uber accident data
That means the official fallback is to contact the responding agency shown in the scene information or case number. If a local police department, sheriff, campus agency, tribal agency, or another agency responded, ask that agency’s records unit about its own copy and request process. If DOR has not yet received the record, use its stated 90-day entry period before treating the state record as unavailable. DMV; CDOT
